Module cover

When a final inspection fails, the real enemy is not the defect but the assumption about why it happened. This module teaches managers to separate symptoms from root causes before acting.

About this module

This module prepares garment factory managers and senior managers to investigate quality failures without blame. You will practise gathering evidence, asking layered why questions, containing the immediate problem, and choosing corrective actions that prevent recurrence.

Learning objectives

  • Separate symptoms from root causes when a quality failure occurs.
  • Apply the 5 Why method using evidence rather than opinion.
  • Set up containment actions that protect the customer while investigation continues.
  • Choose corrective actions that stop recurrence, not just the current defect.
