On a 12-line floor, PP approval for a woven trouser order arrived three days late after a fit comment on the back rise. Line 6 still carried a target of 900 pcs per shift, and the sewing manager pushed the line to start with an unapproved sample as reference.
By the second shift DHU had moved from 8 to 21, mostly back-rise puckering, and 1,150 pcs were held for rework. The delay was outside the manager's control; the decision to start without the approved sample was not.
