## Purpose
Record the evidence and decision that a CAPA has demonstrably worked over an agreed monitoring horizon.

## Required fields
- `capaId`
- `monitoringHorizon` — duration or volume window
- `acceptanceRule` — statistical / operational threshold
- `evidenceRefs` — inspection data, control chart, audit result
- `decision` — Effective / Ineffective / Reopen
- `verifier` — role independent of the action owner

## Rule
Closure without effectiveness evidence is not permitted. An ineffective verdict returns the CAPA to Investigation.
